Spa membership benefits
TBL membership program offers discounts on monthly spa services. The Membership benefits include one service at a discounted rate each month, as well as additional discounts on secondary spa member service(s) within the same month. You will also receive discounts, incentives, and specials throughout your membership term. These specials will vary month to month. Here is how the program works:
Choose a membership type. Based on the service you choose, you will receive one of that specified service(s) each month. You may book an additional service that you have chosen for your membership at the additional discounted rate listed below. Any additional services will be charged at time of checkout.
Fill out authorization form. This will allow us to set up your spa member account. You will be debited for that service the first day of every month.
Facial of your choice plus brow shaping $79/month
Acne treatment $69/month
Body acne treatment $79/month

________ I authorize The Beauty lounge to enroll me in the Spa Membership Program
________ I understand that The Beauty lounge will automatically charge my credit card on the first day of each month.
________ If I do not schedule and redeem my service during any particular month, I understand that I forfeit that month’s service after 30 days. No refunds or credits will be issued for missed appointments.
________ At the end of my 12 month membership commitment, my Spa Membership will automatically revert to a month to month membership. My card will continue to be charged the monthly membership fee.
________ After the initial membership commitment, my month to month membership can be cancelled by turning in a “Spa Membership Request to Cancel” form at least 30 days prior to the next billing date.
________ I understand that I cannot cancel my membership during the 12 month commitment period. If for any reason I do need to cancel before the 12 month commitment period is up, I may do so by filling out a “Spa Member Request to Cancel Form” at least 30 days prior to the next bill cycle and paying a $150 cancellation fee.
________ I understand that I am able to leave without checking out for my service. If I choose to do this, I understand that any services or products will be charged to the credit card on file.
________ As a Spa Member, I will not have to fill out consent forms each visit. I will inform my technician verbally if any new physical or health changes occur that may be affected during my service.
